When the spleen is weak, it can also weaken the kidneys and trigger pain at the base of the tongue, a burning sensation in the mouth, dry lips, hot flashes with night sweating and lower back ache. If blood and qi are insufficient – due to ageing, anaemia or excessive worrying – it will create “heat” and “fire” in the heart, and trigger severe tongue pain, a dry mouth with a burning sensation, constipation with dry stool, depression and insomnia. I visited a doctor also, who suspected my “burning” tongue may be due to gastric acid in the stomach.
Gastroesophageal reflux disease (GERD) occurs when the upper portion of the digestive tract is not functioning properly, causing stomach contents to flow back into the esophagus. The esophagus is a muscular tube linking the mouth to the stomach. In normal digestion, a specialized ring of muscle at the bottom of the esophagus called the lower esophageal sphincter (LES) opens to allow food to pass into the stomach and then quickly closes to prevent backflow into the esophagus. The LES can malfunction, allowing contents from the stomach, including food and digestive juices, such as hydrochloric acid, to push up into the esophagus. Dry mouth, acid reflux, hormonal imbalances, psychological factors like stress and depression, and certain medications can cause this burning or scalding pain. Because burning mouth syndrome is associated with such a wide variety of other medical conditions, your doctor may refer you to a specialist for diagnosis and screening and possibly treatment.
In rare cases, symptoms may go away on their own or become less frequent suddenly. Some sensations may be temporarily relieved during eating or drinking. Avoiding alcohol and smoking and refraining from eating 3 hours before bedtime may reduce the frequency of acid reflux episodes. Treatments that reduce reflux by increasing LES pressure and downward esophageal contractions are domperidone and metoclopramide maleate.

This often leaves doctors thinking that it couldn’t be reflux related, but this problem still stems to LPR where a similar problem exists with proper diagnosis severely lacking and the same applies to burning mouth syndrome. While acid regurgitation from the stomach could produce burning sensations in the chest or the throat, it is not likely to be a major factor in the burning mouth syndrome.
Your health care provider may order an allergy test, salivary or gastric tests, or imaging services even. Burning mouth symptoms can happen in different patterns, as the day progresses like worsening, going and coming, or ongoing symptoms from when you up wake. Though geographic tongue, a condition marked by raised “map-like” appearance of the tongue, may accompany the syndrome, it generally does not cause changes to the mouth.
Having a certain illness or medical condition can also contribute to the risk of burning mouth syndrome. Things to watch for in your medical history include fibromyalgia, Parkinson’s disease, neuropathy, and some autoimmune disorders. Many of these come with nervous system issues, which can play into the syndrome as discussed below.
